The following information will be accepted by the Artists in the Garden Committee until April 30th, 2008
1. Short description of you and your artwork for use on our website (50 word max)
2. Four to six digital images showing all media/styles you plan to display.
3. Two digital images of your choice which may be used on the website and/or in marketing material
4. A post-dated cheque for $100 payable to Hearth Place Cancer Support Centre (dated April 30, 2008) and completed application form

I, the undersigned, have requested and agree to the following conditions of entry:
I will display and sell only my own artwork and reproductions thereof during AITG. I will donate one representative piece of my artwork by May 31st to be used in a raffle on the day of the event. I, the undersigned, for myself, my heirs, executors, administrators and assigns, do hereby waive any claims to which I may become entitled for losses, injury or damage and to hereby release hearth Place Cancer Support Centre and the Artists in the Garden Committee from any and all claims for losses, damages or injury suffered by me as a result of my participation in this event.
